McFarland Clinic is currently accepting applications for a Care Manager (RN) for its Ames office. Candidates should be service-oriented, a team player, and be able to provide extraordinary care, every day to our patients.
Responsibilities include: This role will be split 50%- Care Manager and 50% Immunization/ Access Clinic Nurse. While in the Care Manager role this person will serve as an integral member of the interdisciplinary healthcare team and will be responsible for identifying care coordination needs in collaboration with the care teams, patient and patient’s external support system. While in the Immunization Access Nurse role this person will serve as the professional nurse providing patient centered care providing immunizations and Catheter exchanges following established standards and practices in accordance with McFarland Clinic’s Core Values and Promise.

Roles and responsibilities of Care Manager.
+ The Care Manager provides patient-centered care management services to patients and families.
+ Within the context of an interdisciplinary team, the Care Manager will participate in progressing patients towards discharge/next level of care by identifying barriers and implementing appropriate solutions/interventions.
+ The Care Manager will consistently and thoughtfully apply nursing values and continuous quality improvement in daily work.
+ This position serves as an integral member of the interdisciplinary healthcare team and will be responsible for identifying care coordination needs in collaboration with the inpatient and ambulatory care teams, patient and patient’s external support system (as applicable).
+ The Care Manager works within an interdisciplinary team to facilitate the patient’s plan-of-care throughout the continuum of care by ensuring appropriate utilization management, care coordination, resource utilization, and clinical documentation.
+ The Care Manager will function within the McFarland Clinic Clinical Nursing Professional Practice Model, which includes accountability for assessing, planning, implementing, evaluating, and communicating the patient care plan progression.
+ The Care Manager provides leadership through education to the patient.
+ The Case Manager bridges communication between providers, patients and families, members of the interdisciplinary team, and outside agencies to assure high-quality care that meets the patient’s needs and is delivered in a cost-effective and timely manner.
+ Act as a liaison between patients, health care providers, and health insurers to ensure patients receive excellent health care
+ Maintains patient medical records and keeps track of health outcomes.
+ Consider the patient’s unique care needs and advocate for personalized treatment options.
+ Have exceptional communication skills, organizational skills, and analytical and problem-solving skills.
Roles and Responsibilities for Access Clinic 50%
+ Greets patients and prepares them for immunization or catheter exchange visits, including taking vital signs and updating medication records.
+ Utilizes professional nursing theory and skills during interviewing, observation and nursing procedures and patient/family assessment to identify problems and formulate a nursing diagnosis.
+ Prepares and helps educate patients regarding immunizations and/or catheter exchange.
+ Place orders as directed by physician/provider (as needed).
+ Responsible for documentation necessary information into the EHR.
+ Maintains clinic daily schedule.
+ Maintains exam rooms for necessary supplies and materials.
+ Maintains safe, clean, orderly environment in the office setting.
+ Possesses good working knowledge base of disease processes and treatment modalities.
+ Possesses basic knowledge of current medications/immunizations utilized in the practice.
+ Responsible for assessment, screening, and triaging of Access Clinic patients.
+ Skilled in obtaining accurate patient information by phone and responsible for effective phone triage.
+ Responsible for patient education in all aspects of care.
+ Proficient in scheduling diagnostic evaluations utilizing the various health care facilities.
+ Utilizes sterile technique when establishing a sterile field and demonstrates principles of asepsis when assisting with procedures.
+ Proficient in performing practice specific procedures.
+ Maintains current knowledge of community resources for effective patient referrals.
ADDITIONAL DUTIES AND RESPONSIBILITIES
+ Performs other tasks as assigned by the Population Health Manager in addition to the duties of the Care Manager. These tasks are generally ongoing, require additional knowledge and skills, and reflect a higher level of responsibility.
Education
+ Graduate of an accredited school of nursing.
+ Bachelor’s degree in nursing strongly preferred; would consider Associate’s
Certification/License
+ Current State of Iowa Registered Nurses license
+ Current Basic Life Support certification for Health Care Providers.
+ Mandatory Reporting of Dependent Adult and Child Abuse.
Days: Monday – Friday
Hours: 8:00 AM – 5:00 PM
Experience
+ Prior experience in any of the following areas is strongly preferred: utilization management, case management, home care setup, discharge planning.
+ The schedule is subject to change based upon business needs.
+ Must reside within the state of Iowa.
+ Experience with population health, care coordination, and/or disease management programs preferred.
