+ Position Responsibilities
+ Unit Specific Position Responsibilities
+ Assists in planning and implementing core functions of public health programs, monitors program performance, assists with developing long-term program strategies and goals, prepares program recommendations based on priority health and threats which are identified in community assessments. Evaluates effectiveness of programs.
+ Implements processes to ensure program outcomes are compliant with applicable laws and regulations; prepares reports, assists with budget and monitors expenditures.
+ Develops and maintains appropriate policies and procedures.
+ Meets with the Board of Health to describe and discuss activities.
+ Provides community based health education, primary prevention strategies, care management to individuals and families who are members of vulnerable populations and high risk groups. Gives presentations to community and government groups.
+ Provides oversight and participates in community health clinics, immunization services and audits. Participates in the surveillance of communicable diseases.
+ Performs related duties as assigned.
+ Qualifications, Knowledge & Experience
+ Required Qualifications (Including any licensure, certification, education):
+ Licensed as a Registered Nurse and authorized by the State of Iowa to practice nursing or has active multistate license approved by the Nurse Licensure Compact.
+ Achievement of Baccalaureate degree in nursing within 6 years of hire if hired after January 1, 2014.
+ Basic Life Support (BLS) training, or within 90 days of hire and must maintain throughout employment.
+ Mandatory Reporter training or within 90 days of hire and must maintain throughout employment.
+ Possession of a current valid driver’s license and must maintain eligibility for coverage under Mary Greeley Medical Center’s insurance policy.
+ Organizational Requirements:
+ Maintain stroke education per regulatory requirements.
+ Preferred Qualifications:
+ Baccalaureate degree in nursing.
+ Professional certification in nursing specialty.
+ Community or home health experience preferred.
+ Required Knowledge, Skills & Experience:
+ Proficient in communication skills.
+ Ability to pursue and foster a sense of team and partnership across all disciplines.
+ Ability to communicate effectively, in English, with diverse population groups.
+ Ability to be self-directed, flexible, and able to adapt to change.
+ Ability to communicate effectively in a professional and tactful manner with physicians, staff, patients and/or their representatives.
+ Ability to read and understand chart documentation.
+ Ability to demonstrate basic computer skills.
+ Ability to read, write and understand the English language.
+ Minimum of two years of clinical experience as a registered nurse
